How it works
Six steps, all of them from your phone. Here is what you do at each one and what happens next.
Step 1
Book and pay on the secure telehealth platform
What you do: Choose an appointment time and pay the $150 physician fee on the telehealth platform. Payment is taken there, not on this site.
What happens next: The platform confirms your booking by email and sends you the intake link.
Step 2
Complete your medical forms and upload documentation
What you do: Fill in your medical history and upload proof of your qualifying diagnosis along with your identification.
What happens next: Your submission is reviewed to confirm the documentation is legible and complete before the visit.
Step 3
Your appointment is confirmed
What you do: Nothing, unless something is missing — you would be asked for it at this point.
What happens next: You receive a confirmation with the video link and the time.
Step 4
Join the video visit from your phone
What you do: Join with camera and microphone on, somewhere private, with your physical ID in hand.
What happens next: Dr. Shri Parimi verifies your identification, reviews your documented diagnosis and medical history with you, and makes a clinical decision.
Step 5
The State of Georgia emails you
What you do: Watch for the email after a successful visit, and check your spam folder.
What happens next: The certification is submitted to the Georgia Department of Public Health, which contacts you directly.
Step 6
Pay the state fee and your card arrives
What you do: Pay the state's $30 card fee plus the $3.75 portal service fee through the state's portal.
What happens next: Your Medical Cannabis Card is mailed to the address on file. The card is valid five years; physician certification is renewed annually.
What to have ready before booking
- Acceptable identification. A Georgia driver's license or state ID is accepted on its own; an out-of-state ID requires two proofs of residency. See the full ID list.
- Documentation of a qualifying diagnosis — the clinician note naming the condition, plus the confirming study where one exists. See the 18 qualifying conditions.
- A phone or laptop with a working camera and microphone, and a private place to take the call.
- A payment method for the physician fee, and later for the separate state fee.
