This covers the review of your uploaded documentation, the video visit itself, identity verification, the physician's review of your diagnosis and medical history, and submission of the certification to the state when the physician certifies. This site takes no payment — the charge happens on the booking platform.

$30 plus a $3.75 portal service fee, paid separately to the Georgia Department of Public Health after your certification is approved.
This is a government fee, not a practice charge, and insurance does not cover it. You pay it through the state's portal after the state emails you, and the Medical Cannabis Card is then mailed to you.
A patient who is certified pays $150 to the practice and $33.75 to the state. Those are separate transactions with separate receipts. The card issued by the state is valid five years; the physician certification behind it is renewed annually, which means a further physician visit each year.
