Orthopedic, neurology, rheumatology and pain management practices in the area will release records to you on request, and many now deliver them electronically the same day.

A Georgia driver's license or Georgia state ID is accepted on its own for identification. Documentation of your qualifying diagnosis is the other requirement — the specialist note, the imaging or pathology report, or the laboratory result that names the condition.

If your records are scattered across Wellstar and independent Cobb County practices, you can ask the physician to request records from your treating physician rather than chasing them yourself. Bring what you already have to the visit and discuss the rest.
